Ensuring patients have access to timely and appropriate healthcare requires an effective referral system. Currently, many healthcare providers use manual processes, such as paper-based fax referral forms, which are processed slowly and prone to error. The Ocean eReferral Network is an innovative solution designed to address these concerns by securely sending referrals electronically while keeping the patient, referral senders, and referral recipients informed about the status of referrals.
The Ocean eReferral Network is currently in a pilot phase for three high priority referral pathways in the Waterloo Wellington Local Health Integration Network, including improving access to Diabetes programs, Orthopedic Specialists, and Chronic Disease Prevention and Management resources.
